  • What is included within the period Nov 17,2025 to Jan 17, 2026? - admissions starting Nov 17? or discharges starting Nov 17? or all profiles still in the laptop (complete/incomplete) as of Nov 17? By the end of two months - how to know when to report since each DC has different pacing when entering/completing profiles? Do we care about the criteria for inclusion within the cut-off dates or we care more on collecting these cases as many as we can so we can make decision on the change from our current groupings? Do we want to summarize/report this based on a structure time frame (e.g. admission dates or discharge dates or none)? --JMojica 16:14, 12 November 2025 (CST)
    • We want an overview with as much info as we can get within our window. So should base this on any record we are completing in that timeframe, even if the dispo is some time ago. As I see it, this doesn't have to mean.

Collection of data to help decision of how we will improve Collection of locations on the spectrum between home and PCH.

We know that there are Dispo locations that are sort of between what we traditionally think of as "home" and "regular PCH" like settings, but we don't know how frequently patients are being discharged to them, and what options there are. To get a better idea, we will collect data about these non-traditional locations (NonTradLoc) for 2 months. Include the variations of homelessness and shelter-like accommodations or couch surfing etc.

Data Collection

  • Enter this data for any patient being admitted from or being discharged to a location that you would think of as "not quite home and not quite PCH".
  • Data included: We want an overview with as much info as we can get within the collection window. So collectors should enter this as they come across the info for any record completed in that time frame, even if the Dispo DtTm is some time ago. Don't go back to where you had finished reading the chart but not sent yet because you were waiting for a final detail to send. If you remember that the dispo was to the kind of location we are looking for, then it would be good to include it.
