Data collector's binder

Collection staff port around with their laptops, a data collection binder which contain the follow items:

1. Data collection log forms
2. own history note worksheets per patient
3. completed Greensheets - ICU staff
4. completed TISS76 forms TISS28 (Jan 1.13) - ICU staff
5. Diagnosis code booklets (no longer being published or updated since Nov 2.10)
6. printouts from Wiki if needed
7. Medical records lists
8. other work related items
