"set MRSA Colonized" button

The "set MRSA Colonized" button on the Patient Viewer Tab Dispo in CCMDB.accdb will enter the ICD10 and CCI Picklist entries for an MRSA colonized patient as per Template:ICD10_Guideline_MRSA#Colonization

Background

MRSA is a very common thing, and is somewhat complicated to enter, so we decided to facilitate entering it.
