"generate email" button

The "generate email" button button in the patient viewer form in CCMDB.mdb (between the two close buttons, has an envelope as icon) will open an email pre-populated with some basic data about the patient to make it easier and more consistent when emailing other collector, or others, such as: {{p:Basil Evan]] for CLI or VAP, or Myrna Dyck from ID Infleunza
