Code Palliative Service for Medicine and ICU database program only if the patient is either under the care of Palliative Service Team or that this Service has been consulted. Template:Discussion
- Question Do you code Palliative service if the CTUS4 team consults them ,they answer the consult and makes an assessment and then tells the team the patient does not meet the criteria to be placed on the palliative service. Or do you have to be accepted on the palliative service to enter this code?Just to clarify Thanks !! Or can it be both? Are you wanting data who is accepted into the program or just being consulted with suggestions.????
- I include this code for consults whether they actually accepted or not. A consult requires use of the palliative service and often they give recommendations even if they do not actually enter the program. It is easy to see when they are consulted but if we have to read the consults to see what the outcome is then this is additional work for us. --LKolesar 08:39, 2014 March 6 (CST)
- Since Trish is away I have emailed this on to Dr Garland, who was part of setting up this definition. Ttenbergen 09:39, 2013 December 16 (CST)
Go to: Comfort Care for collection instructions on that care goal.
Coding Instruction for Palliative Care Service
Palliative Service code should only be used as follows:
- ADMIT slots:
- only if the patient has been previously enrolled in palliative care service and they are involved in the patients care.
- when a patient is accepted to medicine or ICU service by attending physician while in ER and also, during this time, the attending physician also writes a request to consult palliative services, then this code can be used in admit DX slot.
- COMPLICATION slots:
- only if there is an actual consultation for Palliative Services after ward admission.
- Does above instruction mean that it doesn’t matter if accepted to service or not, just consulted? What is the purpose of coding Palliative Service consult after admission without knowing if accepted under care or not?
Purpose and Use
