- enter into the "Q" field (note, that's the last field on the form, not the first!)
- no space
- upper case letters
- example only: X0X0X0
- When the postal code is not available, tick off the "B" and leave the space for postal code blank
Relationship between [[Province], PHIN and Postal code
When collecting Postal Code, PHIN number and Province, think about the relationship of this information when you are collecting it.
Special Cases
Homeless with proxy address
If you have a homeless person admitted who lists their address as Siloam Mission or the Salvation Army/Booth's Place, use their postal code as provided.
- Question: I’m just wondering how to manage the postal code of a person with no fixed address and is just couch surfing. There is a postal code listed in the chart b/c pt had been staying at a particular address for some time but may not return to that address. Currently I have checked off no postal code available. I would like your thoughts on the best way to handle the postal code assignment in this situation. Judy K May 11, 2015.

Institutionalized patients
For patients who live in care homes, use the institution's postal code.
Inmates
For inmates such as from Stoney Mountain or Headingley use the institution's postal code.
Postal Code not available
If a postal code is unavailable for a patient, tick off the "B" and leave the space for postal code blank. e.g.:
- John/Jane Doe
- chart doesn't have postal code
- out-of-country patient
- The postal code should reflect the patient’s permanent residence. So for someone from India, the postal code should be not available.
- NOTE: The most common use for postal codes is for researchers to link to Stats Canada for economic status and to for us to get a distribution of patients by MB RHAs outside of Winnipeg. It’s also a cross-check on Province for out-of-province patients. - Randy Martens - Sept 11.15.
Template:CCMDB Data Integrity Checks
Unlike other tmp entries, this entry is not checked as part of the pre-send checker, it is sent when checking TMP complete. PC must be one of the following
- blank postal code, postal code missing check box checked
- postal code missing check box unchecked and letter number letter number letter number
- example only: X1X1X1
province - PC consistency
We used to have a consistency check between province and postal code. It was eliminated because there are weird special cases where consistency between province, PHIN and postal code gave false-positives. The check between PHIN and Province is more important for us, so we eliminated the check for PC vs Province.
