Historically, the data collectors have been provided with a collection worksheet to document procedures and treatment information. This information was scanned in for future reference and for simple clarification of diagnostic coding.
Some locations are still using these sheets, others are using a modified version, and some enter data straight into the PDA.
The collection worksheet can be used to record the pertinent information to help with the following:
- code diagnostic procedure and comorbid information were recorded ad-hoc in an organized point form for coding of diagnoses
- cross checking laboratory, TISS and pharmacy data
- dates of surgery, problems or complications
- transfers from other centers and problems and procedures at the other center

- This is a modified version of outdated information in Trish's manual. Did we ever scan these sheets? Are there other inaccuracies or omissions in this?
