This article provides information on how to request patient charts from Medical Records at HSC. It is part of the HSC ICU Collection Guide and the HSC Medicine Collection Guide.

- Fax Lorna at 75002 or take a print out of the medical record request to her inbox at her desk in medical records.
- charts requested before 1400 hrs will be pulled by MR staff overnight and some should be available the next day as a general rule of thumb.
Audit 59
- used by HSC ICU data collectors
- audit limit is 20-25 charts
Study 780
- used by HSC medicine data collectors
- there is a limit of 20-25 spaces for Study 780
- Medicine Data Collectors should negotiate use of this space if above quota
