Start Date {{{StartDate}}}"{{{StartDate}}}" contains an extrinsic dash or other characters that are invalid for a date interpretation.
End Date {{{EndDate}}}"{{{EndDate}}}" contains an extrinsic dash or other characters that are invalid for a date interpretation.
Program {{{Program}}}"{{{Program}}}" is not in the list (Critical Care, Medicine, not entered, Family Medicine) of allowed values for the "Collection Location Program" property.
Hospital {{{Hospital}}}"{{{Hospital}}}" is not in the list (HSC, STB, GRA, VIC, CON, OAK, not entered) of allowed values for the "Collection Location Hospital" property.
Service Type {{{Service Type}}}Property "Collection Location Service Type" (as page type) with input value "{{{Service Type}}}" contains invalid characters or is incomplete and therefore can cause unexpected results during a query or annotation process.

This template implements a standard Collection Location table and is to be used in the top right corner of all collection locations. It populates the Category: Collection Location and sets the start and end date properties.
