- Office Name: name of the office, must be same as the article name for that office.
- Phone number:
- in case of single number: that number, else leave blank and explain in article after template is used.
- Fax number: Fax number for office, put "outside of office" after number if so
- Room number: just that
- Mailing Address: just that
