This is the original instruction for the PAPER COLLECTION FORM that was used to by the Data Collectors to tally pharmacy data.

original Manual: "Please be careful and collect and total this information in the unit amounts specified on the data collection form. Standard mixtures are dispensed for a variety of drugs at each site. Please be familiar with what they are. i.e. , inotropes

TISS / Pharm corellation

The following items on your pharmacy form should be confirmed against the corresponding items on the TISS:

  • inotropes, heparin, antiarrythmic marked in you history notes and pharmacy form <what TISS items?>

  • Question 1:
  • Are there other lab entries that have TISS or other corellations? We should list them here, and we should include checks for them. Ttenbergen 21:32, 4 June 2008 (CDT)
  • I also check that TISS # 76 or 77 is marked against the antibiotics on the MAR; that Tiss 71 is marked when pt had potassium via central line;#70 is marked if patient is on Vasopressin.

  3. OAKS-ICU-no I do not use paper. My orientation was based on the VIC and the use of the PDA was a method to eliminate paper.
  4. Concordia ICU- I don't routinely use them. I did when I was having a lot of trouble inputting pharmacy(pda kept crashing), but that was only for a very short time. I keep them on hand in case I want them, but haven't used them for quite a long time.
  5. SBGH ICU:no paper, use PDA. Our pharmacy is on the EPR so we can always recheck on the computer if need be, we don't need to pull the chart to do most pharmacy (some exceptions).--LKolesar 13:08, 14 April 2009 (CDT)
