The entry TmpV2 entry for Postal Code collection will be automatically created for any new patient created as of 2011-12-08. It needs to be filled in for any patient, (we will need to decide what to fill in when not available).

  • "Q": enter the patient's postal code (note, that's the last field on the form, not the first!)

Special Cases

Homeless with proxy address

If you have a homeless person admitted who lists their address as Siloam Mission or the Salvation Army/Booth's Place, use their postal code as provided.

Institutionalized patients

For patients who live in care homes, use the institution's postal code.

Inmates

For inmates such as from Stoney Mountain or Headingley use the institution's postal code.

Military Personnel

PC not available

If a postal code is truly unavailable for a patient, enter N/A. Examples:

  • John/Jane Doe
  • out-of-country patient

  • Please fill in how you think we should code this. Specifically, what to do if a postal code is not available, e.g. out-of-country patients, homeless patients, etc. Also, when do you want collection to start? The tmp entry is available now as of 2011-12-08. I will allow empty values for now but will start restricting this once we settle on rules.Ttenbergen 18:42, 8 December 2011 (CST)
