Hi everyone............would like to add this to TMP starting on Friday this week Nov 1.13.  Is everyone now undertanding how to use this TMP file and the new definition/clarification of when or when not to use code Palliative Care.  Let me know the plan is to get this rolling.[[User:TOstryzniuk|Trish Ostryzniuk]] 17:28, 2013 October 30 (CDT)

**Not being ignored, just trying to use the comments and suggestions that are posted and apply them into instructions rather than added to and get lost in discussions.  If the instruction is not making sense, we keep working at it here.  If I don't get to this before the next TASK team meeting it will be on that agenda to help work on it further and where data collection members can speak up there.  Lets see, some good feedback from a number of staff.  Will post another approach shortly.[[User:TOstryzniuk|Trish Ostryzniuk]] 20:24, 2013 October 31 (CDT)

Comfort care, ACP C, palliation

Data Use and Purpose

The purpose of adding this to TMP is allow the medicine program to identify those patients whom the initial intent of care AT the start of a ward admission is: comfort care, ACP C, palliation.

NOTE: Palliation does not mean consult to Palliative Care Service, but can include it.

Collection Instructions

  • Where: ccmdb TMP:
    • PROJECT: Admit Intent (current suggestion for project name)
    • ITEM: Comfort Care
  1. Ward admitting physician/resident's order/note is required to be present on the chart at the time of ward admission stating: that intent for this ward admission is for: comfort care or ACP C or palliation.
    • feedback from a number of staff is that we should not be relying on Doc notes because the collector see the reality that a note this specific is not often there.
  2. Admit DX's would use the same process as we always do. Most significant Medical problem in admit 1, 2, 3 etc.

Note: comfort care, ACP C, palliation all mean that the INTENT of care at the time of ward admission is for comfort care. This does not preclude having some medical treatments like treating an pneumonia or infection, pain, dehydration and anything else that would ease suffering.


Shirley Kiesmans question: pt comes to CTU/S4 with a Dr.'s note/order stated intent of care is for comfort care/ACP C. Pt has Dx of CVA, CAP, cancer.

  • Admit DX would be: 1. CVA 2. CAP, 3 CA which ever one is the primary problem first. Could be CAP because had CVA last week.
  • TMP
    • Project: Admit intent
    • Item: Comfort Care
  • Item Comfort Care: I would like to suggest having a definer added to "Comfort Care as an /End-of Life Care --Llemoine 12:54, 2013 November 4 (CST)
