get rid of the diagnosis error list line

Sorting on Patient LIST Form
Need to be able to sort by, LN, FN and may a third option if needed. Only sorting by one column and not grouping it by first name. This is a problematic for processor to find a same patient in the the patient list form.
- discussed with Pagasa, will combine as "<LN>, <FN>"
D_ID EDITS
D_ID cannot edit. Var 1 and Var 2 no downlist to edit
S_Locations table
