This article provides information on how to request patient charts from Medical Records at HSC. It is part of the HSC General Collection Guide.
Contact info
- Fax Lorna Sasley at 75002 or take a print out of the medical record request to her inbox at her desk in medical records.
- charts requested before 1400 hrs will be pulled by MR staff overnight and some should be available the next day as a general rule of thumb.
Shelf 59
- used by HSC ICU data collectors
- audit limit is 20 charts
This is specific to HSC ICU as space is extremely limited!
Shelf 780
- used by HSC medicine data collectors
- limit of 20 charts at any given time
- Medicine Data Collectors should negotiate use of this space with each other, especially if one collector has many charts in medical records.
Shelf 1007
- used by HSC - main office - ONLY for special audit done by physicians or ICU quality officer related to ICU or Med Database.
- Must be authorized by Database Manager Trish Ostryzniuk for purpose listed above.
- limit of 20 charts
- Obtained on: December 14, 2015 and approved HSC Medical Records manager.
