Causes and solutions

If any records are listed

• delete items from L TISS Item table
  • open L TISS Item table
  • filter by D_ID
  • sort by Item_Date
  • delete all those records that have dates of the duplicated form
• delete record from L TISS Form table
  • same as above...
• re-scan the duplicated form

Could fix these one by one, but usually it's quickest to just re-scan.
