Location profile

Non-acute Medical/Geri rehab unit, patients are almost always medically stable with discharge planning, examples include PCH, Geri-regab, or HC. Should these patients develop acute medical problems, they are discharged from S3.

Template:Discussion is that following right? Answer is Yes

The unit is split between Medicine and Family Medicine services:

  • S3 - we collect
    • 14 beds allocated for VMU/CTU Medicine patients - medicine patients we collect
    • Room numbers 301-1 to 318-4
  • S3 Family Medicine - we do not collect - if patient is from/to here, enter as Vic Ward
    • and 14 beds allocated for Family Medicine patients (FM patients are not entered into the Database).

Collection info

Unique to S3 is a census sheet that is maintained/updated by hand to assist with the tracking of admissions/discharges as the S3 A/D/T log book is often incomplete. The census sheet is periodically cross referenced with the S3 Unit Addressographs and the date this is done recorded on the census sheet. The S3 Census Sheet is kept in a folder along with the N5/S5/S3 Data Collection Log.
