Index numbers

The index number was used in the main office solely for form filing purposes only. It was abandoned when we started collecting PHIN number. Any paper files are now filed by Phin.

We no longer use this reference number for any purpose. When Julie refers to Index number, she is referring to her own index number that she assigns in her SAS programs.
