Black blobbing

Black blobbing is the filling in of circles on the TISS form completely with a soft pencil (HB or B) or black pen or marker. Pens with coloured ink should not be used. Properly filling in the bubbles, i.e., staying with the circle boundary and using the recommended pencil or pen will ensure the form is read correctly by Teleform.

Before being submitted for processing, completed forms should be checked for the quality of the marks. Very faint or incomplete marks should be fixed before submitting.
