"Make Service" button

The "Make Service" button on the Patient Viewer Tab Cognos ADT2 in CCMDB.accdb will turn that Service Start Dtm of and Service name into a Service tmp entry. The dropdown of whether this is a move to boarding location or to home ward still needs to be filled in manually.
