HSC Medical Records requests

This article provides information on how to request patient charts from Medical Records at HSC. It is part of the HSC ICU Collection Guide and the HSC Medicine Collection Guide.

ICU

Fax number 75002 Lorna is in charge of Audit 59.The audit limit is 20 charts shared between HSC medicine data collectors and ICU data collectors.

Lorna reminded us that when referring to Medical records for chart pulls: Refer to AUDIT 59.(July 16,2013) Thank you. That is their system.

Medicine

• requests are written then may be faxed to Lorna at 75002 or a print out of the MR list can be hand delivered to Lorna and placed in her incoming mailbox.
• charts requested before 1400 hrs will be pulled by MR staff overnight and should be available the next day as a general rule of thumb.
• there is a limit of 20-25 spaces for Study 780 and Medicine Data Collectors should negiate use of this space if above quota.
• ICU Audit 59 has approx. 20 spaces available
