Data collector's binder

Collection staff port around with their laptops, a data collection binder which contain the follow items:

1. Data collection log forms
2. own history note worksheets per patient
3. completed Greensheets - ICU staff
4. completed TISS76 forms. Will be TISS28 on Jan 1.13 - ICU staff
5. Diagnosis code booklets (no longer being published or updated since Nov 2.10)
6. printouts from Wiki if needed
7. Medical records lists
8. stickies for medicine OverstayProject
9. other work related items
