Don't put paneling as the first diagnosis if a patient has acute problems and has been transferred to your ward for treatment of those problems. For example if a patient on a chronic unit has been paneled but gets HAP and transfers to your ward then the #1 diagnosis for your ward is HAP.This was discussed with Dr Garland at the task group meeting on March 1,2013.Trish Ostryzniuk 18:49, 2013 March 8 (EST)

Template:CCMDB Data Integrity Checks

  • If Admit Diagnosis 1 Diagnosis code is 9060* then:
    • Transfer Ready Date and Time must = Admit Date and Time to your unit.
      • Tina to implement new check since the old one was deleted with the original code. Ttenbergen 10:49, 2012 October 1 (CDT)
        • Putting this on hold for now, since this Dx will likely go away.

DX coded as Paneling but discharge home

Note: there are cases that ARE newly paneled while in hospital, however, the pt is discharged home. This is because the family decided they could manage with home care, or they will take pt home and continue to wait for nursing home placement. (as per feedback from collection staff. • STB has a new Medicine Program Paneling Approval Process. Using the DPST and the regression program, a high risk patient is identified early and paneling can be initiated early. There is also a subset of patients where high risk is not as obvious, and where the ability to go home is uncertain. It is these patients in which a decision is made to panel from the hospital, but do not necessarily need to be paneled as an inpatient. Once all assessments are made and staff feels that the patient should be paneled, the decision is presented to the Panel Approval Committee, who decides whether paneling could be done from home or as an inpatient. If decision made is to panel from home, this is the only option presented to the Patient/Family So, yes, it is correct that I have admit dxs. Or complications of panel and then the patient is discharged to home. Elaine April 15.13
