CCMDB.accdb links to CCMDB wiki

Most of the fields in CCMDB.mdb link to the CCMDB wiki. This provides immediate access to documentation on how, exactly, to use a field. This is useful as part of the Critical Care and Medicine Database Core Curriculum, but also to stay up to date, and to make it a little bit more likely that nuanced instructions may be followed.

Implementation details

Currently this is implemented by making every field label a link. Collectors have provided feedback that these links often get clicked on accidentally, slowing down collection.

• I have changed the label for DOB to no longer be a link and instead have a link icon next to it. How does that work for everyone? I'd like to get this right for one field before I update the others, because changing all fields again later would be tedious. Thoughts?
