GH-SWAP Location

The EPR/Cognos unit entry GH-SWAP Location is used when a pt is swapped form one bed to another. There is no physical SWAP location, and is used in EPR when they are changing patient bed/unit assignments. Similar concepts exist elsewhere.

Collection Instruction

• We don't enter Swap locations, but will enter the move in the Boarding Loc if the unit has changed. If someone remains in the same unit but moved rooms, nothing is entered.
• In CUS, it will show up under the Unit as GH-SWAP
  • These entries can sometimes remain in the CUS even after you've updated the patient's boarding location. If you see it staying in the CUS list, confirm the entry is correct (location, date/time) and if everything is correct clear it from your list using the "exclude" button
• You will see one location entry going to GH-SWAP, and then another entry to follow with the patient moving from GH-SWAP to their new bed/unit location. They are usually kept for <5 min in the SWAP location
  • Use the Unit Start date/time of the new boarding location when entering in COGNOS
• For example, in COGNOS you will see location entries as follows:

   2022-01-19 12:15 to 2022-01-21 21:25 in GH-EMERGENCY
   2022-01-21 21:25 to 2022-01-22 09:30 in GH-3SOUTH
   2022-01-25 12:00 to 2022-01-25 12:01 in GH-SWAP
   2022-01-25 12:01 to 2022-01-26 16:00 in GH-5NORTH

This patient was admitted to 3 South from ER and when they transferred the patient to 5 North they had to temporarily (in EPR) add the patient to the SWAP location before finally completing the transfer (in EPR) to the new boarding location on 5 North.

Swaps within same unit

This can also happen when a patient needs to move rooms on the same unit (for example from a regular room into an isolation room). You'll see the patient entries as listed above, only the new location after the SWAP entry will be the same as the location they were coming from. In this case, nothing new to enter because their physical location has not changed.

Background

• In the 40 days preceding 2020-12-15 there appear only 2 of these entries and they are about 1 minute each.
