The lab collection form was used to tally daily the labs whose counts we collect for the entire LOS] in ICU. All counts were summed up prior to handing in the form for manual data entry by the data processors. This form is double sided at some sites, where so an arrow was used to indicate if collection was continued on the back. If more than one form was used, the totals for all forms were included on the last page only
