By the end of two months - how to know when to report since each DC has different pacing when entering/completing profiles? Do we care about the criteria for inclusion within the cut-off dates or we care more on collecting these cases as many as we can so we can make decision on the change from our current groupings ? Collection of data to help decision of how we will improve Collection of locations on the spectrum between home and PCH.

We know that there are Dispo locations that are sort of between what we traditionally think of as "home" and "regular PCH" like settings, but we don't know how frequently patients are being discharged to them, and what options there are. To get a better idea, we will collect data about these non-traditional locations (NonTradLoc) for 2 months. Include the variations of homelessness and shelter-like accommodations or couch surfing etc.

Data Collection

  • keep in mind that we are trying to better understand what is out there, so if the item is not on the list, but is an odd curve ball that might be relevant to organizing discharge, that might be even more important to include than the items on the list
