We use a form called green sheet to get the ward attending doctors in the medicine program and at the VIC and STB ICU to tell us
- admit, transfer and discharge times
- most important admit, complication and comorbid diagnoses
Since there are inconsistencies in filling these out, we are compiling stats about their completion in the L_Green table.
It is the data collectors responsibility to remind site attending physicians to complete these form.
