It is for discharged patients from HSC med wards to Lennox Bell Lodge. These are not patients on hospital hold but are individuals that need ''interim assisted housing'' for a ''short period of time''. Data Collectors do not need to continue to follow these patients once discharged to Lennox Bell.
Lennox Bell Place (LBP) is a short-term housing placement option for individuals who are in hospital, medically stable, ready for discharge AND they have an identified reason that prevents them from returning to permanent housing at this time. The WRHA is able to offer a transition unit to such individuals on a short term basis (up to 90 days) while the person secures appropriate housing or health and social services are arranged.
 
Data Collectors do not need to continue to follow patients when discharged to Lennox Bell.
