Query created PatientFollow

The query created_PatientFollow in CFE contains a definition of whether the record was set up under the rules following PatientFollow Project or before. The concept is used in many places so a single query was implemented to make sure this is done consistently everywhere.

• 2022-09-06 - changed definition from PatientFollow_old: (Nz([Accept_DtTm],[Arrive_DtTm])>#2020-10-15# Or [Dispo_Dttm]>#2020-10-15#) to because the old definition left records with accept/arrive before but dispo after without boarding locs.
