Lennox Bell Place (LBP) is a short-term housing placement option for individuals who are in hospital, medically stable, ready for discharge AND they have an identified reason that prevents them from returning to permanent housing at this time. The WRHA is able to offer a transition unit to such individuals on a short term basis (up to 90 days) while the person secures appropriate housing or health and social services are arranged. An application form is required before acceptance to LBP.

Data Collectors do not need to continue to follow patients when discharged to Lennox Bell Place.
