The lab collection form was used to tally daily the labs whose counts we collect for the entire LOS in ICU. All counts were summed up prior to handing in the form for manual data entry by the data processors. This form is double sided at some sites, where so an arrow was used to indicate if collection was continued on the back. If more than one form was used, the totals for all forms were included on the last page only
Quote manual: "Data for laboratory and tests to be collected from a treatment sheet kept at the front desk or can be counted off a computer or from the patients’ chart. Each site will vary, but a process will be established to make it as easy as possible for the collector. Please be careful with CBC diff tallying. The diffs we want you to collect are the manual ones not the automated ones. If you don’t know what to look for PLEASE ASK. The costs associated with a manual differential are much higher than the automated ones. "
