This is a temporary project to track source collectors use to abstract when a patient is accepted by Medicine or ICU service from the Emergency Department.

Operation Planning - Patient Flow. Region Managers and Director are monitoring and tracking patient flow from the 'emergency department' to their unit. The data helps support any changes needed to improve patient flow from ER to unit.

We currently track Accept Date/Time however, we need to identify the "source" that this information is being abstracted from by collectors in Region. We are starting a short term project in order to do this.

Template:Discussion before we start, I need help for the drop down list for items in regards what the sources might be. This is tentative list subject to staff input. I have started the list below:

  1. Doctors order
  2. History notes
  3. EPR
  4. ICU admission notes
  5. ER form - Request for Admit to inpatient unit
  6. Guess - no documentation
  7. Inpatient summary sheet (this is a pink sheet at the VIC)
  8. ICU consult that indicates acceptance
  9. Standard ICU order sheet
  10. Emergency doctors notes indicating ICU has accepted the patient this may be on EPR at some sites.
