Sometimes patients are admitted under a stolen identity, and occasionally this is noticed and affects the patient's chart.
Collection Instructions
- Put the real name if available with the real DOB, real [[Chart number] and Phin number in the Database entries and put the stolen ID in the alias spots.
- If other admissions were recent and part of our database, contact the collector or main office to sort this out as an alias for previous admissions. Also let Pagasa know if the wrong name was initially used in the profile.
