ESRD vs Acute renal failure

• Do NOT code both
  • Chronic kidney disease (end-stage renal/kidney disease, ESRD), Stage 5, GFR LT 15 AND
  • Any of the acute renal failure diagnoses, even if the person with the Chronic kidney disease (end-stage renal/kidney disease, ESRD), Stage 5, GFR LT 15 is not yet on dialysis, as that code includes GFR<15 mL/min even if not on dialysis yet.
  • UNLESS the patient has had a renal transplant and the transplanted kidney was functioning (and thus can experience acute renal failure)

  • Chronic kidney disease (chronic renal insufficiency, uremia) Stage 2, GFR 60-89
  • Chronic kidney disease (chronic renal insufficiency, uremia) Stage 3, GFR 30-59
  • Chronic kidney disease (chronic renal insufficiency, uremia) Stage 4, GFR 15-29
