Location profile

  • Medicine unit.
  • Occasionally Oncology patients are admitted for the prime reason of receiving Chemo, but we do not put these patients in the data base.

This information has been added to the exceptions in Definition of a Medicine Service admission.

Unit admission log book/Transition Coordinator Binder

The admit/transfer/discharge binder is located at the main desk and is kept between the CRN and the Ward Clerk. Please return binder to this spot.

The transition coordinator binder is located directly behind the Unit Clerk at the main desk.

Collector "base"

There are four computers in the Nurses' conference room behind the main desk area and another four computers at a small desk area at the furthest end of the hall.

Location of patient chart components

Charts are in "rolling carts" main desk area. Discharge charts may be found in a wire basket in the Doctors' Charting room. The basket is on the upper right hand shelf over the computers.

Colour dots for Overstay project may be found on one of the shelves over the patient charts, if not, they are kept in a large pull-out, lower drawer to the right of the main desk.

Thinned/old charts are kept in a grey filing cabinet in the Dr's charting room behind the main desk

Historical Information

STB B5, E6 story

See STB_B5 start date. (STB B5+STB E6)=STB E6.

October 14, 2009 all new new admits on this day change location to E6.

  • Any new patients admitted to STB_B5 on Oct 14th onward will have LOCATION of E6. (starts at serial number 6311 Gayle’s PDA and serial 4598 Deb’s PDA for B5 patients).
  • Any patient admitted to B5 Oct 13, 2009 or earlier will retain LOCATION of B5. (Last B5 patient serial 6310 (Gayle PDA) & 4597(deb’s PDA)

Move dates:

  • If the B5 admit date was “on” or after Oct 14, 2009 then moves data should be to E6.
  • If the B5 ward admit date was October 13, 2009 or “earlier” then move data is B5 even if patient is move to you on or after October 14, 2009

(as taken from elsewhere to prevent duplication, not integrated yet)

  • STB_B5 - location label changed to STB_E6.
    • Change date: Oct 14.09 @ 0001 hrs (all patient's admitted or transferred into to STB_B5 starting October 14.09 @ 0001 hrs onward will have location, move or var 1 or 2 changed to STB_E6).
      • (stop at serial 6310)
      • (stop at serial 4597-Deb's PDA every 86-100.
        • Once Gayle has sent in all patients that were admitted to B5 prior to Oct 14th, then STB_B5 in S_Location table will be removed.
  • STB_E6-change date: Oct 14.09 @ 0001 hrs.
    • (start new location at serial 6311)
    • (start at serial 4598 -on Deb's PDA every 86-100). Stop using these serial numbers for E6 in March 2010 when Elaine was assigned to E6. These serial numbers should no longer be used for E6 patients.--TOstryzniuk 21:35, 8 July 2010 (CDT)

  • If the admit date of the other ward was October 13, 2009 or “earlier” then var 1 should be FROM B5.
  • If the admit date of the other ward was “on” or after Oct 14, 2009 then var 1 should be FROM E6.
  • If the discharge date of the other ward was October 13, 2009 or “earlier” then var 2 should be TO B5.
  • If the discharge date of the other ward was “on” or after Oct 14, 2009 then var 2 should be TO E6.
