somewhat useful

  • HSC_MICU - The green sheets are usually filled in for HSC MICU. I find them useful.I always read them and they help with coding.Gail Hall
  • CON MICU - green sheets are usually filled out. I find that many of the comorbids found on the chart are not on the green sheet. The "final diagnosis" is often written on the first day or two of the pt's admission. If the diagnosis changes after further diagnostics; I find that the "final diagnosis" is not updated/corrected/changed to reflect this. Most complications are filled in; but all are found on the chart. I do review the green sheets and find them only somewhat useful.Mlaporte 13:09, 2013 November 6 (CST)

minimally useful

  • STB_MICU - At times I refer to it only if the diagnosis is unclear. The green sheets are only filled out by certain attendings and most of the time it is a waste of paper because everything can be derived from the chart. --LKolesar 13:35, 2013 November 5 (CST)
  • STB_CICU - I find the green sheets are poorly filled out.Probably about one quarter are not filled out.The information I want which is the complications is never filled out.--LBilesky 13:47, 2013 November 5 (CST)
  • The green sheets are not filled out in HSC CCU as often as MICU. Gail Hall

  • STB_CCU - Marla Penner- The Green sheets are mostly blank in my unit. I don't rely on these sheet for any of my documentation due to poor compliance from the CCU Drs.--mvpenner 13:49, 2013 November 5 (CST)
  • HSC_IICU - The green sheets are not filled in well for HSC IICU and they are often lost as they are sent with the chart to medical records.Gail Hall

The Diagnosis "Greensheet" is used by the Critical Care Physicians in the Region. The information on the form provides some of the data we collect and supports data collectors efforts.

Data on Greensheet

  1. primary admitting diagnosis
  2. complications (problems, procedures and surgery) that occur after unit admission (also referred to as "acquired diagnosis").
  3. Comorbid conditions
  4. Admit, Transfer and Discharge date and time
  5. Discontinuation of Treatment

Data Collector Responsibility RE: Green sheets

  • ICU & CCU Collection only
  • remind site attending physicians to complete these form.
  • record Serial number on each form
  • ensure addressograph info on each form
  • collect and submit paper form to main database processing office for each patient that is discharge from unit
    • before submitting to office organize forms from lowest to highest serial number order
    • clip together and attach a TAG with the following information:
Site, Location, Program, batch sent number, initials: (Example: HSC D4 MED Batch 10 BB)

Green Sheet Compliance Data Stop Date

There used to be data collection and tallying by our collectors to keep track of physician compliance. This is no longer done as of August 20.09. The Statistician has the compliance reports from the greensheets.mdb.

Medicine Green Sheet Stop Date

As of December 7, 2009, after email directions from Dr. Roberts, greensheets are no longer facilitated or processed on medicine units by the medicine database program.
