Data collector's binder

Collection staff port around with their laptops, a data collection binder which contain the follow items:

1. Data collection log forms - ICU and medicine collectors
2. own history note worksheets per patient - ICU and medicine collectors
3. completed Greensheets - ICU collector
4. completed TISS76 forms. Will be TISS28 on Jan 1.13 - ICU collector
5. Diagnosis code booklets (no longer being published or updated since Nov 2.10)
6. printouts from Wiki if needed
7. Medical records lists
8. colored stickies for medicine OverstayProject - Medicine collector
9. other work related items
