Family Satisfaction in the Intensive Care Unit
The Survey - Family Satisfaction in the Intensive Care unit (FS-ICU) is a survey that the ICU Quality Team would like to run annually for three months.

Data entry
Karen, the admin person for Linda Hathout's Quality and Process Team, did the manual entry in an Access.mdb.
Data Exporting
Fred Schumacher did the exporting from Access and submitted it to Julie Mojica
Data Analysis
History
The designed Teleform survey form was lost for some unexplained reason in 2011 when moving Teleform program to newer computer. For some reason it could not be recovered from old computer either. That computer is RIP as of 2011.
