The Green Sheet form is a bright green colored paper form that is used in Critical Care and on Medicine wards in the Region to obtain "required" participation in the Database Program from the attending physician for the following:

  1. primary admitting diagnosis
  2. complications (problems, procedures and surgery) that occur after unit admission (also referred to as "acquired diagnosis").
  3. Comorbid conditions
  4. Admit, Transfer and Discharge date and time
  5. Discontinuation of Treatment


Data Collector Responsibility RE: Green sheets

  • remind site attending physicians to complete these form.
  • record SERIAL number on each form
  • ensure addressograph info on each form


  • collect and submit paper form to main database processing office for each patient that is discharge from unit
    • before submitting to office organize forms from lowest to highest serial number order
    • clip together and attach a TAG with the following information:
Site, Location, Program, batch sent number, initials: (Example: HSC D4 MED Batch 10 BB)

  • STB ICU I think there is more to the greensheets issue than just if they are filled out and to what extent. The question is, how useful is this information. I think we should do an audit on greensheets for a certain time frame and see if the data collectors find the information on the green sheets of any value. Tina, perhaps this could be put in the temp. file? EXAMPLE: How useful was this green sheet information? Very helpful; Somewhat helpful; Not helpful . After this type of audit we could then evaluate if the green sheets are worth keeping. What does everyone think?--LKolesar 09:14, 7 April 2009 (CDT)
    • GRA-MED-In all honesty, I do not rely on the green diagnoses forms as there are too many differences in our coding definitions vs. physician definiton i.e. ARF vs. ARI, Hyperkalemia etc. Even transfer ready date & time is not reliable. SCortilet 13:02, 7 April 2009 (CDT)
      • At the Vic ICU the green sheets are filled out very sporadically. We also may have a Doc write in an adm diagnosis of respiratory failure for example. This is not helpful & cannot be used. We have recently put the green sheets in front of the med orders which has only very marginally increased compliance with them being completed. Frankly after 10 years of this we no longer hound the Docs to fill them in. NMiller NMiller
      • To clarify, VIC MICU did collect green sheet data on PDA or CCMDB. Paper forms are mostly marked blank! Very few physicians fill them out. Norine--Tue 11/3/2009 8:41 AM
        • At the Oak in ICU there is very poor compliance rate in completing these forms. Wendy Turner, 23, April, 2009 1400 hrs.OAK ICU Question: These green sheets, which I realize were never collected with SOGH,what do they do and how are they contributing to DATA? Reason I ask as the Docs are HORRIBLE AT FILLING THEM OUT PROPERLY. I refuse to hound them, I speak with them, leave messages and that's it.           • At Concordia ICU the Dr's are actually pretty good at filling them out. The binder is on the rounds card and they fill it out as they review each pt. I have found it helpful at times, as they will write in dx that never make it into the chart (because the charting is done by the CA's). I don't bother them about filling them in anymore...I did at the beginning though. I'm not really sure why we have such good compliance here...perhaps our medical director has something to do with that. BDeVlaming 09:38, 4 November 2009 (CST)

  • Subject: ABOLISHING Database Program GREEN SHEETS for Medicine Units
  • The attempt to have CTU attendings fill the diagnostic summary “green sheets “ on the GIM units has been largely unsuccessful. It is costly in terms of processing paper and the data collection nurses by and large find them unhelpful. We will therefore proceed to abandon their use over the next week on all CTU’s and NTU’s .
  • If you feel the diagnostic summary is useful for the caregivers at your site , (since on some units it is used more consistently then others), please feel free to continue but the sheets will no longer be linked to the data collection process and your site will continue to bear the cost of producing the forms.
  • Note that the sheets will continue to be used in the Critical Care units for the time being.

        • Green sheet PAPER FORMS are still to be collected and handed as all sites as we have been doing.
  • We are stopping the Green sheet Accounting on the “PDA” and Access for medicine wards and for STB ICU.
  • The HSC_ICU, OAK_ICU, VIC_ICUand GRA_ICU never collected this info on their PDA.
  • Please see attachment that shows what I am referring on PDA and Access that will stop
  • Tina, for STB ICU, and all medicine wards (STB, HSC, GRA and VIC), will sending be interrupted if these medicine units stop greensheet accounting on PDA or in Access? Will Access require a page to be created in order to send or have you disabled this for these sites?
  • Dr. Robert has made this part of the physician job description that they must participate by documenting on these forms. You can email me names and I will take it up with Dan.--[[User:|TOstryzniuk]] 09:05, 3 November 2009 (CST)
