Overstay2 processes on the units to reduce discharge delay

Processes

See 110.500.411, Discharge Planning Screening Tool (DPST)

Broadly:

• The CRNs will review the Overstay2 Screening Reports and record the result in Medworxx Go section
  • for boarding patients, Utilization will do it
• Red pts will be "followed" by a Utilization Specialist to provide enhanced support for discharge planning and coordination.
• US will review chart and document detailed assessment of risk in IPN
• Pts will be added to the Complex and Long Length of Stay (LLOS) tracker
• there will be a weekly interdisciplinary review, led by the Utilization Specialist, will be conducted for all patients categorized as “Red” as well as others identified as having an anticipated complex discharge.
