Only patients who are ventilated in hospital can have a complication dx of VAP.
If a patient has a complication VAP and no TISS28 items T18 or 19 then this is an error.
Implemented as query NDC_VAP_no_TISS.
This doesn't take into account CPAP/BIPAP.
