Template to make sure extra information is consistent for the Category:Decubitus ulcer diagnoses corresponding to the template name.


Stage Not Indicated

  • use this code when it was not clear from the charting what stage the ulcer is

  • This instruction is taken from old dx Decubitus (pressure) ulcer, stage not indicated; since we will now have a series of codes for suspected deep tissue injury, we need to change this, but what should be here, and what should move into Template:Decubitus Deep Tissue Damage Details?       • This template had an evil twin "Template:Decubitus Stage not indicated Details" that wasn't actually linked, I cleaned it up and moved this question from that template to here. This template is linked from all unstageable decube pages. I am concerned that the instructions that were brought over might mislead someone to coding a deep tissue injury as "stage not indicated". Possibly all it takes is to remove that, just want to make sure it doesn't need to be replaced with something else. Ttenbergen 01:46, 11 March 2026 (CDT)

  • Unstageable - the depth is unknown- full thickness tissue loss in which the base of the ulcer is covered in slough and/or eschar
  • Suspected deep tissue injury- depth unknown- purple or maroon localized area of discolored intact skin or blood-filled blister due to damage of underlying soft tissue.
  • includes necrotic pressure ulcers where the stage is not indicated

This code covers two situations:

  • When the stage presumably is known by somebody, but was recorded and so the data collector doesn't have the stage available to them
  • When it is "unstageable" from a technical standpoint, i.e. can't see the wound bed.
