Stage Not Indicated
- use this code when it was not clear from the charting what stage the ulcer is

- Unstageable - the depth is unknown- full thickness tissue loss in which the base of the ulcer is covered in slough and/or eschar
- Suspected deep tissue injury- depth unknown- purple or maroon localized area of discolored intact skin or blood-filled blister due to damage of underlying soft tissue.
- includes necrotic pressure ulcers where the stage is not indicated
This code covers two situations:
- When the stage presumably is known by somebody, but was recorded and so the data collector doesn't have the stage available to them
- When it is "unstageable" from a technical standpoint, i.e. can't see the wound bed.
