Stage not indicated / unstageable

Unstageable - the depth is unknown- full thickness tissue loss in which the base of the ulcer is covered in slough and/or eschar
Suspected deep tissue injury- depth unknown- purple or maroon localized area of discolored intact skin or blood-filled blister due to damage of underlying soft tissue.

	This instruction is taken from old dx Decubitus (pressure) ulcer, stage not indicated; since we will now have a series of codes for suspected deep tissue injury, we need to change this, but what should be here, and what should move into Template:Decubitus Deep Tissue Damage Details? This code covers two situations:

When the stage presumably is known by somebody, but was recorded and so the data collector doesn't have the stage available to them
When it is "unstageable" from a technical standpoint, i.e. can't see the wound bed.
