{{DiscussAllan | How is this different than: [[Stroke, NOS]]?}}
== Additional Info ==
*This code is for '''cerebral ischemia/stroke due to thrombosis''' of a cerebral artery (not embolic).
'''Includes'''
* Lacunar infarcts on old imaging or documentation

*If the ischemic stroke due to thrombosis spontaneously '''"transforms" and bleeds''', then COMBINE it with the appropriate code for the hemorrhage (see below).
*If the hemorrhage after an ischemic stroke was NOT spontaneous, but followed anticoagulation, then the hemorrhage should be coded as [[Hemorrhage, due to anticoagulant or thrombolytic drug]] as a complication of treatment [[Iatrogenic, complication of medical or surgical care NOS]] not a complication of the stroke per se.

'''Excludes'''
*If the ischemic stroke is '''embolic''' (including cardioembolic), use '''[[Cerebral infarction/stroke due to embolism]]'''.
*If a stroke is '''not specified''' as hemorrhagic, embolic or ishemic, use '''[[Stroke, NOS]]'''.
=== Comorbid Rule exception ===

*If the ischemic stroke is embolic (including cardioembolic) then instead of this code, use '''[[Cerebral infarction/stroke due to embolism]]'''.
*If this code occurs, and then the initially ischemic stroke spontaneously "transforms" and bleeds, then COMBINE this code with the appropriate code for the hemorrhage (see below)
**However, if hemorrhage after an ischemic stroke was NOT spontaneous, but followed anticoagulation, then the hemorrhage should be coded as '''[[Hemorrhage, due to anticoagulant drugs]]''' since it's not a spontaneous hemorrhagic transformation, and thus is a complication of treatment, not a complication of the stroke per se.

Comorbid Rule exception

Regular comorbid rules exception: Strokes (of any sort, ischemic or hemorrhagic) include any residual deficits by linking with the stroke code -- code even resolved strokes with no current sequelae as comorbid diagnoses. If a patient has a past history of stroke, use the regular stroke codes to identify this, even if the patient does not have any residual deficits we will now capture ANY past history of stroke. This is contrary to the usual rules about Comorbid_Diagnosis#When_not_to_code_a_dx_at_all.
