Historically, the data collectors have been provided with a collection worksheet to document procedures and treatment information. This information was scanned in for future reference and for simple clarification of diagnostic coding.
Some locations are still using these sheets, others are using a modified version, or their own version. A few staff directly enter data straight into CCMDB.accdb on the laptop without using any paper history notes.
The collection worksheet can be used to record the pertinent information to help with the following:
- code diagnostic procedure and comorbid information were recorded ad-hoc in an organized point form for coding of diagnoses
- cross checking laboratory, TISS and pharmacy data
- dates of surgery, problems or complications
- transfers from other centers and problems and procedures at the other center
