• This code is for new creation of a tracheostomy -- it may be done in the OR or at the bedside using a bedside trach kit, or a true open procedure
• It can be coded more than once only if the tube falls out and the hole closes up, and as a result the procedure needs to be done again, but do NOT code this if all that happened was the tube fell out and somebody came back and put a new one into the existing stoma.
