This article provides information on how to request patient charts from Medical Records at HSC. It is part of the HSC General Collection Guide.
Requesting charts
- Email Lorna Sasley or take a print out of the medical record request to her inbox at her desk in medical records.
- Charts have to be assembled before being pulled, and due to the backlog in MR, they will not be available until the following month --Jvelasco 08:56, 2020 February 20 (CST)
- if charts are from iron mountain, they will take 2 days.
- For chart review collectors may use the Research and Analysis room (where we normally collect), or if space is too limited for social distancing, there is also another area with separate cubicles that can be utilized. Inquire at the front desk for details.

Shelf 59
- used by HSC ICU data collectors
- limit of 25 charts at any given time in this slot
- this is specific to HSC as space is extremely limited!
Shelf 780
- used by HSC medicine data collectors, split between A4 and D4
- limit of 25 charts at any given time in this slot
- Medicine Data Collectors should negotiate use of this space with each other, especially if one collector has many charts in medical records.
Shelf 1007
- used by HSC medicine data collectors, split between H4 and B3/WRS3.
