If an ICU or Medicine patient expires and the chart is no longer available on the unit it has likely gone to the HSC Death Registry or Death Desk. The death desk is on the main floor beside medical records.
In the case of ICU patients, once that desk is finished with the chart they will often be found on the transplant audit shelf, located on the left hand side in the hallway in MR.
