Important note: Data Collection Paper Patient Logs

Data collector must use black or dark blue pen only. Do not use a pencil! When pencil is used information on these forms are faint and therefore not visible when faxed or scanned. Thank you.--Trish Ostryzniuk 14:45, 2012 May 7 (CDT)

  • I thought you no longer wanted our log forms, as we are supposedly paperless. I thought logs were our personal preference for tracking our own patients. --FLindell 12:53, 2012 May 8 (CDT)

Comments

We have been using pencil in our log sheets for a long long time because we are always needing to change times and we erase a lot on these sheets. In critical care I think most data collectors send a copy of the log sheets with their tiss and greens via mail system so they are not usually faxed or scanned. Does this apply to all data collectors or just those who scan or fax their log sheets?? --LKolesar 07:24, 2012 May 8 (CDT)

Transfer Tracker

I have changed the Transfer Tracker.mdb to :

  • list patients who have not yet been discharged
  • increase list of patients available to delete pts with discharge dates more than 7 weeks old to better accommodate vacations
